First Visit

Brief Paperwork

Upon entering the office for your first visit, we have some paperwork for you to fill out. The paperwork provides us with a detailed history about you and your condition.

Please complete new patient forms for chiropractic, there is no need to print, simply complete and hit submit!

Consultation

Once the paperwork has been completed, you will have a complimentary consultation with the doctor to discuss your health-related problems, concerns, as well as potential treatment options.

History and Examination

In order to determine a diagnosis or further action with regards to your health, the doctor will ask you various questions related to your case. Next, a number of specialized tests will be performed to determine which tissues are affected and to what extent.

X-ray Studies

Your specific presentation may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Schedule Your Next Appointment

You will be given an appointment time for your follow-up visit. This visit is known as your report of findings, and will be your longest session at our clinic; should you be accepted for care, this appointment will be followed by your first adjustment. Treatment options for your condition will also be discussed during this time.
